That checkout belong the Woocommerce plugin.
I normally, if you setup checkout page and cart for Woocommerce it will work good.
I’ve test with the order of “tester”, it wirk good.
For the second attachment, it look like a issue from woocommerce.
Could you please send us your login details and FTP details in private reply that we can check.
